Who Can Enter: Limited to
legal residents of the 50 United States and
Washington DC, age 18 or older as of April 1,
2008. Minors under the age of 18 must obtain the
consent of their parent or legal guardian in
order to enter the contest. Only one registrant
per household is accepted. All federal, state,
and local laws and regulations apply. Void where
prohibited or restricted.
By
entering this Sweepstakes, participants agree to
be bound by these Official Rules and the
decisions of the Sponsor and/or its agents. Odds
of winning prize depend on the number of
eligible entries received. BY

How to Enter: Individuals
receive one entry into the DogCancerVet.com
Survey Sweepstakes by completing the
registration form after they take the survey on
DogCancerVet.com. Only one registration per
household is accepted.
For those individuals not interested in taking
the survey, they may enter the sweepstakes by
sending a postcard with their full name, postal
address (including: street, apartment/suite
number, city, state and zip code), phone number,
and email address to: DogCancerVet.com Survey
Sweepstakes, c/o Maui Media LLC, 2463 S. Kihei
Road C-16-109, Kihei, Maui, HI 96753. Only one
registration per household is accepted.
The mail-in entries
must be postmarked April 1, 2008 through June
15, 2008, and must be received no later than 5
p.m. Hawaiian Time June 30, 2008 (the "Eligible
Mail-In Entries"). Only one entry per household
per day is accepted. Any mail-in entry
improperly dated will be disqualified. Any
mail-in entry that is illegible, incomplete,
late or does not carry proper postage will be
disqualified.

Sweepstakes Dates: The
DogCancerVet.com Survey Sweepstakes will begin
on April 1, 2008. The prize drawing will be held
based on number of sweepstakes entries through
June 30, 2008 (log ins & registrations through
June 30th plus Eligible Mail-In Entries).
Subsequent sweepstakes may be conducted in the
future. There is no carry-over of sweepstakes
entries from one sweepstakes period to the next.
We may conclude the sweepstakes program at any
time.
4.
Prizes: The total retail
value of all sweepstakes prizes is $100.00.

Prize Drawings: Winners
will be selected at random by Maui Media, LLC.
based on entries obtained via log-ins &
registrations from April 1, 2008 through June
30, 2008 and Eligible Mail-In Entries. The prize
drawing for the sweepstakes will be held on or
around July 15, 2008.
6.
Odds of Winning: Each
entrant's odds of winning depend on the
entrant’s number of entries via log-ins &
registrations from April 1, 2008 – June 30, 2008
and Eligible Mail-In Entries relative to the
total number of entries received via log-ins &
registrations from April 1, 2008 – June 30, 2008
and Eligible Mail-In Entries.
7.
Winner Notification: The
winner will be notified via e-mail within 24
hours of the prize drawing (on or about July 15,
2008). Winners will be required to respond to
the e-mail to acknowledge receipt of the prize.
If no response is obtained within 7 calendar
days of notification, attempts will be made to
reach the winner by phone or mail. If no
response is obtained from the winner by July 30,
2008 the prize will be awarded to a replacement
winner drawn at random. Maui Media, LLC's
decisions about winners are final. Prize is not
transferable.
8.
Limits of Liability: Maui
Media, LLC has no responsibility whatsoever for
injuries, losses, or damages of any kind that
result from acceptance, possession, or use of
any prize. Maui Media, LLC assumes no liability
for lost or misdirected registration forms or
survey submissions whether due to technical or
computer malfunctions or otherwise. Maui Media,
LLC assumes no liability for technical or
computer malfunctions of any kind. Maui Media,
LLC reserves the right to cancel the sweepstakes
at any time if the either the DogCancerVet.com
website or the SurveyMonkey.com website is
infected by a virus, the website is hacked or
the integrity of the sweepstakes is compromised
in any way.
9.
Change to Rules: These
Official Contest Rules are subject to change
each month that the sweepstakes is conducted.
Participants should review the Official Contest
Rules each month.
10.
Winner Responsibilities:
Winners are solely responsible for the reporting
and payment of any tax liabilities incurred.
Winners must submit an affidavit of eligibility
and compliance with the Official Contest Rules.
11.
Winner Publicity:
DogCancerVet.com and/or Maui Media, LLC may wish
to publish the first name, first letter of last
name, and location (city/state) of winners, and
any quotation that the winner is willing to
provide on our websites. However,
DogCancerVet.com and Maui Media, LLC stands by
their commitments of confidentiality related to
personal identity and will request written
permission from each winner prior to publicizing
any names on the website.
